Following consideration of the stress of surgery, RV function should be ascertained. This is graded 1–5 from minimal risk (1 point) up to refractory shock (5 points) (Fig. 2). The sum of the stress of surgery (0–2) and the baseline level of RV function (1–5) generates the cumulative POQI 9 RV Risk Score, which has 5 levels. The higher the level, the greater the potential for escalating care requirements (see Fig. 2)
